In consideration of the payment of the sum of $1.00 and other valuable consideration herein acknowledged as received and sufficiency of which is hereby acknowledged by me, I ____________ hereby grant to Photographer, their heirs, legal representatives and assigns, those for whom Photographer is acting, and those acting with their authority and permission, the irrevocable and unrestricted right and permission to copyright, in their own name or otherwise, and use, re-use, publish, and republish photographic portraits, pictures, motion video, or sound of me or in which I may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations, in conjunction with my own
or a fictitious name, or reproductions thereof in color or otherwise, made through any medium and in any and all media now or hereafter known for illustration, promotion, art, editorial, advertising, trade, or any other purpose whatsoever in perpetuity. I also consent to the use of any printed matter in conjunction therewith.
I hereby waive any right that I may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.
I hereby release the Photographer, their heirs, legal representatives and assigns, and all persons acting under their permission or authority or those for whom they are acting, from any and all claims and demands arising out of or in connection with any such use, including without limitation, any and all claims from invasion of privacy, infringements of my right of publicity, defamation (including libel and slander), or any other cause of action arising out of the taking of said picture and from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said picture/video or in any subsequent processing thereof, as well as any publication, production, distribution, broadcast, exhibition or other exploitation thereof. I also release you from any and all claims and demands arising out of or in connection with any illness or injury I may incur from my involvement in the shoot.
I agree not to sell or offer for sale publication rights or allow commercial use of any photo provided by the Photographer without the Photographer’s prior written consent. The Photographer grants to the Model the right to use any Photo provided for the making of personal prints for use in personal portfolios or personal promotions of a non-commercial nature. In all cases I will credit the Photographer and Photographer’s website and when posting to social media I will credit the Photographer, Photographer’s website, and the Photographer’s
social media account.
I hereby warrant that I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.
